After reading through several forums and reader comments on sales sites, it's seems as if there are issues with all media. Which companies actually make quality media?

How can you tell where ity's from (i.e. japan or taiwan)?

From Japan, Ricoh or TY seem to be very good.

Ricoh is mainly made in Taiwan by Ritek however....

But yes, Taiyo Yuden is VERY good.

I just took advantage of a half price sale at Best Buy: 50% off FujiFilm =D> .
100 Disc for $43.48 out the door, and they are Taiyo Yuden (Japan), code TYG02.
I have compared them to Ritek (Taiwan), Silver Shiny G04 and Verbatim which is manufactured by Mitsubishi Chemical Corp. (Japan) , coded MCC 01RG20. I have found all of these disc to work great. I have burned a copy of Last of the Mohicans on each to compare these three, and I will be dammed if I can tell any difference in the quality. Maybe in a hundred years there will be a difference. I look for a good price, and name brand media. Also, I use a Plextor 708A for all my DVD-R burning. :o
