Where can the audio burning process fail?

When using Exact Audio Copy to rip a CD in secure mode, where can the process create an imperfection in the duplication? It seems like the DAE quality of the drive wouldn't matter because the secure mode makes sure it's correct.

Then once you've got your perfect CD image, how can imperfections be introduced in the burning of it to a CD-R? It seems like high jitter would be one problem. What else could go wrong?
